Well...this sure made me drool over homemade bread, I have not made any in a long time. But some think the kneading techniques are the reason for the holes...not rolling and flattening the bread for the proper size pan can effect the holes. HMMMM....who cares, if it tastes good...yummmy.

MY ANSWER:

Actually, its a combination of things.
First of all, these holes wouldn't form at all
if it wasn't for the "gluten" in the bread.

Gluten is a protein found in flour.

When dough is kneaded, these proteins stretch and become rigid.
Gas gets caught between these rigid structures.
Like everyone else said,
yeast (which is a fungus) produces CO2 as a waste product.
CO2 isn't the only gas that forms the holes.
Steam is also produced when the bread is baked.
Also, baking hardens the gluten structures so the holes cannot collapse.
