I was wondering that myself.

When I was doing research on the rope, I read one commentator that said when Nadab and Abihu dropped dead for offering profane fire in their incence burners before God, they had done it in the Holy of Holies, but I'm not so sure he was right about that. The altar of incence wasn't in the Holy of Holies. It was in the Holy Place.
