On February 3, 2023, about 8:54 p.m. local time, eastbound Norfolk Southern Railway (NS) general merchandise freight train 32N derailed 38 railcars on main track 1 of the NS Fort Wayne Line of the Keystone Division in East Palestine, Ohio.1 (See figure.) The derailed equipment included 11 tank cars carrying hazardous materials that subsequently ignited, fueling fires that damaged an additional 12 non-derailed railcars. First responders implemented a 1-mile evacuation zone surrounding the derailment site that affected up to 2,000 residents. There were no reported fatalities or injuries. At the time of the accident, visibility conditions were dark and clear; the weather was 10°F with no precipitation.

......The positive train control system was enabled and operating at the time of the derailment.

Train 32N was operating with a dynamic brake application* as the train passed a wayside defect detector on the east side of Palestine, Ohio, at milepost (MP) 49.81.4 The wayside defect detector, or hot bearing detector (HBD), transmitted a critical audible alarm message instructing the crew to slow and stop the train to inspect a hot axle. The train engineer increased the dynamic brake application to further slow and stop the train. During this deceleration, an automatic emergency brake application initiated, and train 32N came to a stop.5
On the Fort Wayne Line of the Keystone Division, NS has equipped their rail network with HBD systems to assess the temperature conditions of wheel bearings while en route. The function of the HBD is to detect overheated bearings and provide audible real-time warnings to train crews. Train 32N passed three HBD systems on its trip before the derailment. At MP 79.9, the suspect bearing from the 23rd car had a recorded temperature of 38°F above ambient temperature. When train 32N passed the next HBD, at MP 69.01, the bearing’s recorded temperature was 103°F above ambient. The third HBD, at MP 49.81, recorded the suspect bearing’s temperature at 253°F above ambient.
NS has established the following HBD alarm thresholds (above ambient temperature) and criteria for bearings:
  • Between 170°F and 200°F, warm bearing (non-critical); stop and inspect
  • A difference between bearings on the same axle greater than or equal
    to 115°F (non-critical); stop and inspect
  • Greater than 200°F (critical); set out railcar
* On a diesel-electric locomotive, dynamic braking uses electric traction motors as generators, slowing the train and dissipating mechanical energy as heat.

NTSB investigators identified and examined the first railcar to derail, the 23rd railcar in the consist. Surveillance video from a local residence showed what appeared to be a wheel bearing in the final stage of overheat failure moments before the derailment. The wheel bearing and affected wheelset have been collected as evidence and will be examined by the NTSB. The vinyl chloride tank car top fittings, including the relief valves, were also removed and examined by the NTSB on scene. The top fittings will be shipped to Texas for testing under the direction of the NTSB.

Interesting. Neither track nor brakes involved.

That said, it raises the question of what the procedure is for such an occurrence, and if the engineer knew. There may be an automated system that notifies the engineer of detected problems.

It seems spacing of the temperature detectors (HBD's) was insufficient to provide sufficient warning to the crew.

East Palestine ‘controlled burn’ could have been avoided, NTSB chair says

The Norfolk Southern train derailment in Pennsylvania again came under scrutiny at a Senate hearing.​

Jennifer Homendy, facing questioning from Sen. J.D. Vance (R-Ohio), testified that contractors employed by the Norfolk Southern railway company “lacked the scientific background” to decide that a vent-and-burn was necessary to head off a chemical reaction that could cause the cars to explode. She testified that a better option would have been to allow the tank cars time to cool down.

As part of its ongoing investigation, the NTSB has disclosed that Gov. Mike DeWine (R), Fire Chief Keith Drabick and other officials were told they had just 13 minutes to decide whether to vent the vinyl chloride-carrying cars or wait and risk a likely uncontrolled and catastrophic explosion.

“They were provided incomplete information to make a decision,” Homendy said Wednesday. “There was another option: let it cool down.”

Homendy also said that experts from the OxyVinyls chemical shipping company informed Norfolk Southern’s contractors that they didn’t believe the chemical reaction was taking place or could take place, but that they were not included in the meeting where the decision to do the vent-and-burn was made.

After the hearing, Vance said her testimony raised questions about whether the railway company or its contractors were recommending the vent-and-burn to “facilitate the rapid movement of freight,” given that trains were able to pass again soon after the burn.

Norfolk Southern and its contractors have denied the allegation, saying that the only way to keep people safe was to do the vent-and-burn.

Norfolk Southern reaches $600 million settlement to settle East Palestine derailment suit


Norfolk Southern has reached a $600 million settlement that, if approved by a court, will resolve all lawsuits covering thousands of residents within 20 miles of the 2023 East Palestine, Ohio, derailment that spilled more than a million pounds of hazardous chemicals into the soil, water and air.

The rail company said the settlement is intended to offset costs related to the spill that sent a plume of toxic smoke into the air and displaced many residents and businesses. But Norfolk Southern didn’t admit to any liability or wrongdoing as a result of the settlement.

Feds reach $310M settlement with railroad over East Palestine train derailment

Norfolk Southern will pay a $15 million civil penalty and cover the costs of cleaning up the site of the toxic disaster.

Under the agreement, Norfolk Southern will pay an estimated $235 million to cover the costs of the Environmental Protection Agency’s efforts to clean up contaminated air, water and soil in and around where the train derailed and where toxic fumes were later vented. In addition, the railroad will pay a $15 million civil penalty to resolve alleged violations of the Clean Water Act.

The $15 million civil penalty is unlikely to satisfy residents of East Palestine who had called for the railroad to face stiffer punishment. But Regan said it is the highest amount allowed under the Clean Water Act, given the facts of the case and the alleged violations.

[This settlement with We The People is different from the settlement above with the class action suits from residents.]

Over the past year, the nation’s five largest rail operators together spent roughly $17 million to lobby lawmakers, while donating generously to key members of Congress who oversee transportation issues, according to federal records. Some of the chief beneficiaries of industry cash were Republicans, who initially attacked the Biden administration over its handling of the East Palestine derailment before opposing or slowing down safety legislation.

In doing so, rail industry lobbyists also fought the Biden administration on even the most basic upgrades, from efforts to ensure that engineers have special breathing equipment onboard to new rules that would require miles-long trains to be staffed with more than one person. The staunch opposition has bogged down some federal action while leaving Congress unable to hold a single vote on rail safety legislation in the House or Senate.

East Palestine train derailment spread 'hazardous' pollution to 16 states: Study

In the study published Wednesday, researchers analyzed rain and snow water samples collected at 260 sites from surrounding states the week of the derailment (Jan. 31) and the week after (Feb 14).

"Our measurements revealed a large areal impact from the Midwest through the Northeast and likely Canada, and perhaps as far south as North Carolina," according to the study.

Researchers found that 19 sites had at least one chemical compound in the 99th percentile, while eight sites had four or more compounds in the 99th percentile.

"The impacts of the fire were larger in scale and scope than the initial predictions, and likely due to the uplift from the fire itself entraining pollutants into the atmosphere," researchers wrote.
