It's been four years. None that I've heard of.

I don't know how many teachers want to fight that battle. You have to choose your battles, I had a situation where I was "corrected" and just said nothing. My Aunt was high up in the English Department at Berkeley, she had tons of books in her office because it was her job to select books for the students--I wonder what she would think. I don't think all schools have a formal policy, here is an example at the University of San Francisco and for Berkeley.
