People who pull out in front of me (to the point of making me slam my breaks on) and then go 15 mph below the speed limit.
This is particularly infuriating when the person is driving a brand new V6 or V8 vehicle which is more than capable of doing the speed limit, even up hill.
Also, people who choose not to use their turn signals, but expect me and every other driver on the road to know through ESP when and where they are going to turn.

People who drive heavy equipment/construction equipment/farm equipment that goes 10 mph on a busy street with a 45 mph speed limit and won't move off the road to let the mile-long line of vehicles behind it pass.
