You should be demonstrating a committment to change, and to achieving a healthy lifestyle so that the organ is accepted and you get the most use out of it. If one person is unwilling to do this, and because of this attitude, they have a high probability of suffering complications, it is, I think, the most moral decision to place them at the bottom of the list.
