Prodromos gave a good link on the prosphora. As far as how to make the imprints, you typically can borrow the prosphora seal from your priest or parish when making the bread.

And if you would like to own a seal for yourself you can buy them from Orthodox stores online.

Also check to see if your parish holds classes to teach you how to make it. My parish does.

Know your oven!!! I made prosfora once and it came out lopsided because I forgot that my oven didn't bake evenly, that it was tilted.

If you know how to bake bread you know how to make prosphora. Obviously there are no preservatives or dairy added. You can purchase a prosphoro seal from any online Orthodox book/ suppky store.
