And I will kill her children with death; and all the churches shall know that I am he which searcheth the reins and hearts: and I will give unto every one of you according to your works.

And I looked, and behold a pale horse: and his name that sat on him was Death, and Hell followed with him. And power was given unto them over the fourth part of the earth, to kill with sword, and with hunger, and with death, and with the beasts of the earth.
The word translated as kill is Strong's G615 - apokteinō or ἀποκτείνω - meaning put to death, kill, or slay.
The word translated as with is Strong's G1722 - en or ἐν - meaning in, by, with etc.
The word translated as death is Strong's G2288 - thanatos or θάνατος- meaning death or deadly.
Some have said this refers to pestilence or disease, but those words were readily available and used elsewhere in the NT, so I think that is rather unlikely. Does anyone have an idea what this could mean?
