If your child, with two healthy arms, came to you and said they felt they needed a third arm, would you support surgery to have an arm attached?

It’s likely that there is a common neurophysiological variant causing both gender dysphoria and BID (Body Integrity Dysphoria.) The linked study indicated that BID patients, compared to a control group, were less likely to identify with their anatomical sex. But BID is worse. Affected persons will self mutilate themselves in order to force amputation of whatever limb they believe is foreign. An intense desire to be paraplegic is another variation. So in answer to the OP, elective limb amputation may sometimes be necessary to prevent a worse outcome. Just as gender confirmation medication and surgery can be necessary.

I'd say they came in a pair so they got to be removed as a pair, can't just take one and not the other.

It would depend on if the extra arm removal was a hackneyed analogy for sex reassignment surgery or not.
Im going to assume not. So I'd like to pursue the question of arm removal on its own terms.
