A little game theory would prove that nuclear war is actually a significant possibility, but I'll give you a historical example. In the Cuban Missile Crisis, a Soviet sub approached the American blockade. It had been given permission to deploy nuclear warheads against American targets. The tensions inside the sub rose as the Americans rained depth charges on it. Finally, the captain snapped and ordered a nuclear strike on the American carrier claiming that they were at war, but his co-officer refused.
