I don't do well if I set an alarm... so I set my appointments for late morning and early afternoon (the ones I go to for ME!)... If I have an early appointment I end up not sleeping because of the anxiety of worry that I won't wake and function well enough to make it. 
I live with chronic pain. I figured out a system of using my pain meds that let's me get about 3-4 hours of sleep... at least... anything above that (being able to go back to bed, sleep) is a good night's sleep
I also need a short nap in the middle of the afternoon, or I don't sleep that night. Sigh. Go figure, I'm too tired, pain wired, muscle spasmed to sleep without it.
