What exactly is the difference between kaos and smilies?

A kao is an image like this:

There are many different styles, colors and shapes, but the thing they have in common is that they are a puffy, cloudlike shape, that can be 1, 2, or 3-dimensional.

A smiley is an image like this: or

. It, as you can see, is a smaller, usually circular and one-dimensional image.
