Technically it doesn't, 3 of the mountain peaks are a range outside of the city itself. Multiple cities are actually built on 7 hills, and have those sites inside the city limits today, but Jerusalem isn't one of them.Jerusalem sits on seven hills.

The actual default was the Rome of the First Century. That was what John was referring to when he wrote Revelation. At the time he wrote Revelation, Jerusalem was already destroyed and was basically a ruin, so he could not have had Jerusalem in mind. It's not a nuclear explosion and you can't hide in caves and around rocks to be saved from that anyways. The passage says what they are hiding from:

Rev 6:13 And the stars of heaven fell unto the earth, even as a fig tree casteth her untimely figs, when she is shaken of a mighty wind.
Rev 6:14 And the heaven departed as a scroll when it is rolled together; and every mountain and island were moved out of their places.
Rev 6:15 And the kings of the earth, and the great men, and the rich men, and the chief captains, and the mighty men, and every bondman, and every free man, hid themselves in the dens and in the rocks of the mountains;
