Tough call. I would have to say either Spiderman 2: The Game (for PC) or any of the Armored Core games.

Wizardry 8 wasnt half bad, certainly not the best I've seen but it was still fairly decent.

Worst, I'd have to say Universal Combat.

Transformers for the NES. As soon as you start the game you basically have to rely on luck to survive.

Space Channel 5 for Dreamcast. Michael Jackson is in there - nuff said.

There are MANY Sega Genesis titles that fit in that category.

I played Bard's Tale for the PS2 and liked it alot. I thought is was pretty funny

For worst game ever, I nominate Metal of Honor: Underground for the Gameboy. A great PS1 game made into an absolute horror. Of course, there are a lot of bad games made for the Gameboy system...

Metal of Honor is one of the most highly rated and most popular of games for the PS1.... but, to each his own I guess

Mechwarrior 2150 for the SNES. But mechwarrior is an awesome series right? Right, Mechwarrior 2 and 4 mercs on PC were awesome. Mechwarrior 2150 was a confusing game where the enemies just keep coming and you weren't going to win. It's only recently I found out I meant to buy Mechwarrior for SNES, the clearly superior title.

And the Angry Nintendo Nerd on youtube shows you the truly awful games, like Roger Rabbit and Back to the Future.
