Of course God can discern if we're taking medicine for legitimate purposes or trying to get high & get drunk??
Even courts of law seek to find out motive - if something was an accident or premeditated. Intent is everything.
You know before ether came along, people used to drink whiskey & get stone drunk before medical procedures to dull the pain. Do you don't think God doesn't know the difference?
