The Bible says not every sin leads to death, but some sins do. The Bible also says some sins will not be forgiven. Forgivable sins are not = to unforgivable ones.

And can we assume these sins are done on accident out of ignorance? Because who would dare do something unforgivable to God if they knew it.

Here are the ones that come to mind.

1. Blasphemy against Holy Spirit. (Attributing something as the Holy Spirit when its not, or saying wrong things about it?).
2. Revelation 22:18 (Taking a verse out of context, or using a wrong Bible?)
3. Idolatry (If someone believes Apollo Quiboloy is Jesus that's Idolatry and Blasphemy).
4. False Gospel (Preaching a false Gospel or different Gospel.)

These are the 4 most dangerous? And what do all of them mean to you?

Number 2 above I don't see as an issue.

The Roman Church has perhaps the most clearly defined teaching on mortal vs venial sin. The writer of 1 John 5:16 likely had in mind those sins of the old testament requiring the death penalty.

What is Revelation 22:18 really? I'm sure it applies to the entire Bible.

Revelation condemns those who re-write the book. The gnostics of the time had the annoying habit of re-writing most any book they got their hands on to advocate their own beliefs.

The Gospel of Thomas is believed to be an authentic Christian writing, though with gnostic interpolations.

The writer of the Revelation didn't want his to end up the same.

I have been told that the unforgiven sin is the unrepented one.

You've got it all perfectly right.

It's why the Mormons are hellbound, even though they say the believe in Jesus, it's because they believe in a false Jesus preached from a false Gospel. :(

If you want to know what sins are imto.death and what sins are not, read the books of Moses. It records the law completely.

Re blasphemy of the Holy Spirit: Yeshua said it to the Pharisaic rulers and teachers of the law who were required to have thorough knowledge of the law and the prophets before they could be teachers. One cannot commit that unpardonable sin without the clear knowledge that one is wrong and persisting in defiance of God's word. They hardened their hearts against Him in spite of His clear demonstration that He was of God. Instead of rethinking and repenting, they dug their heels in. They asked that His legs be broken while He was on the cross because they knew the prophesy that not a bone of Messiah would be broken. They were well versed in Messianic prophesy, and rejected Messiah because He threatened their power system. They saw Lazarus raised from the dead, and instead of repenting, they plotted to kill Him. Blasphemy of the Holy Spirit requires knowledge of His work and willful opposition to it.
