This may not necessarily be a religious topic, but I need guidance nevertheless. My friend has been acting weird now for over a year. She is just completely obsessed with becoming fat and all of the stuff that goes with it. She is 4'11 and weighs 115 lbs. And she thinks that she is just sooo fat.

Tonight, I asked her if she had been making herself throw up after meals and she said yes. She has done it a total of 12 times since Tuesday. I just don't know what to do or say. She told me not to tell anyone. I have been praying for her for awhile now and I told her so. She just said that she knew what she was doing. But she really really doesn't. I do not know what to do......

Encourage her to get help right away. This is dangerous behaviour. Vainity and envy can destroy our lives and make us always unhappy. We should be happy with good health and the body God has given us - even being ugly and fat is better than lying in a hospital bed.
Maybe you could get at the root of the problem with her in conversation and encourage her to find her worth and beauty in God and not in what the world idealizes as beauty. If we are ungrateful to God for the bodies He has given us in life, we just live terribly unhappy lives, always envying others.

1Co 13:4 Love is patient; love is kind; love is not envious or boastful or arrogant

Ro 1:29 Their lives became full of every kind of wickedness, sin, greed, hate, envy, murder, fighting, deception, malicious behavior, and gossip.

You need to know that what she is doing could kill her if she doesn't get help. Ask her to tell an adult - a parent or teacher or guidance counsellor or pastor or doctor or nurse - about this and get herself help. Let her know how concerned you are about her. Tell her that you'll go with her for moral support if she wants. If she refuses to tell anyone on her own, then you need to go tell someone. By doing so, you just might be saving her life - and saving a friend's life is being a better friend than keeping a secret that will kill them.
