One would think that if any lesson is to be learned from the Terry Schiavo case, it would be that every adult should have a written AD. And by all means, if you do want tube feeding, or any other kind of life support--put it in writing. And it's crucially important if you think there may be conflicts among your family members regarding end-of-life issues.
