I remember signing my name right before the shot. They'd given me papers with vax information, including risks and side effects, even death. There was no reason for me to be ill-informed.

We are always given the sheets for each vaccine (the official cdc sheets printed for each one) and we have to initial on a sheet next to the name and serial # of vaccine in our kids' medical records. This is the way we've always been asked to do it.

Yes, I'm given the info sheets and I have to sign the vax record that I was given the info and that I consent to the vax.

Wow that's crazy.... I've never signed anything or initialed anything. They do give me the info sheets to take home.

Well I'd imagine that it's because of atrocious people who get their child immunised without reading the info sheets or whatever, then their child has a reaction (mild or serious) and they sue the doctor for "not telling them".

We just sign saying that we know the vaccine is being given. They do give us a sheet that says what the risks are, but we don't have to sign a paper saying that we have read it. I'm sure it is just a way of protecting themselves against any lawsuits that might come up.
