Have they ever had you sign a form that says these are the risks to vaccinations? They have not asked me to sign such a form for either of my children.

Yeah same thing for me.Yes, every time we get one. Just to say "yes, I have received and read the risks etc". No big deal, it's just part of the procedure.
its 18 mnths and then 5 yrs...just before K...(some may be 4 turning 5)...however it is done the yr the child enters K-We receive an information sheet detailing the ingredients of the vaccine , possible side effects and how to treat the minor side effects ( fever or sore injection site) , the sheet also explains that you are to stay in the waiting room for 15 minutes after the innoculation so that the nurses are nearby if there is a serious reaction ( anaphylactic). The nurse always explains these things as well as giving me the sheet to hold onto for reference. They ask if there are any known allergies or reactions to previous shots or ingredients in the shots.
When the baby is very little - like 2 days home from the hospital - so , about 4 -5 days old- ( when the nurse visits you in your home to check on you and baby) there is a consent and asgreement saying you intend and agree to vaccinate your child up to the 18 month vaccine. I understand our vaccine shcedule is a litle different then in the U.S. Here we vax at 2 months , 4 months , 6 months , 12 months , 15 months , 18 months ~there might be one at 24 months? can't quite remember~ and 4 yrs. If I recall correctly in the U.S. children receive their first vax while still in the hospital? Like only a matter of a day or two old?
That's the way our doctor does it too.We are always given the sheets for each vaccine (the official cdc sheets printed for each one) and we have to initial on a sheet next to the name and serial # of vaccine in our kids' medical records. This is the way we've always been asked to do it.
