Many many years ago I had to sign a form to receive vaccinations for my babies... But they quit even giving us the possible side effect sheet of paper back when my 9.5 yr old was a baby (we selectively vaxed him). I had to wonder what reason they had for not even telling parents there Might be side effects...

But to go back to your original question, Yes, I did have to sign a form to receive vaccinations~ but for my oldest kids (now 25, 23, 18, 16, 14...) not any of my youngest.

We are always given the sheets for each vaccine (the official cdc sheets printed for each one) and we have to initial on a sheet next to the name and serial # of vaccine in our kids' medical records. This is the way we've always been asked to do it.

We receive an information sheet detailing the ingredients of the vaccine , possible side effects and how to treat the minor side effects ( fever or sore injection site) , the sheet also explains that you are to stay in the waiting room for 15 minutes after the innoculation so that the nurses are nearby if there is a serious reaction ( anaphylactic). The nurse always explains these things as well as giving me the sheet to hold onto for reference. They ask if there are any known allergies or reactions to previous shots or ingredients in the shots.

When the baby is very little - like 2 days home from the hospital - so , about 4 -5 days old- ( when the nurse visits you in your home to check on you and baby) there is a consent and asgreement saying you intend and agree to vaccinate your child up to the 18 month vaccine. I understand our vaccine shcedule is a litle different then in the U.S. Here we vax at 2 months , 4 months , 6 months , 12 months , 15 months , 18 months ~there might be one at 24 months? can't quite remember~ and 4 yrs. If I recall correctly in the U.S. children receive their first vax while still in the hospital? Like only a matter of a day or two old?

its 18 mnths and then 5 yrs...just before K...(some may be 4 turning 5)...however it is done the yr the child enters K-:)

and then in grade 5 and now grade 9...

For some reason though I had one in gr 12...whether its because I missed my gr 9 I dont know...:scratch:

they are doing mumps right now as its going around...or was last yr anyways for those in University and College...

I know when I had the flu shot I had to wait for so long (abt 10 min) before leaving to make sure I wasnt allergic...and had to sign the form for myself as well that I understood the risk...as I said, since kids cant understand we do that for them...and last time dh did that...:)

Have they ever had you sign a form that says these are the risks to vaccinations? They have not asked me to sign such a form for either of my children.
it's required by law here. and information pamphlets have to be passed out. it may just depend on where you live.

I'm thinking that since my kids have been vaccinated at military clinics, they don't bother with the signing because we have no real rights anyway.

You should see what the injected my husband with before he left for Iraq :eek:

We sign a consent to vaccinate and then a paper with the lists of side effects (that we get a copy of). We also sign something on the chart saying that we either did or didn't consent to that vaccination (the only one we didn't consent to was MMR, so I signed the chart saying I din't let them give it to him nd then another paper saying the same thing....wasn't badly worded or anything, just saying that I understand that without the shots, he is susceptible to the illnesses).
