I'm adding actual reconstructions of the master-copy behind the lost ancestor of the two related manuscripts Sinaiticus and Vaticanus (Aleph/B). (these are compliments of mr.scrivener).

Here you can see a variety of possible formats that caused the scribe of the lost ancestor of Aleph/B to skip a line and lose some text. Over 50 of the omissions of Aleph/B in the NT are a result of this basic mistake, caused by losing one's place on the master, while copying.

Here again we can see an obvious homoioteleuton error waiting to happen in the traditional text, and it did, in the Alexandrian stream. There are many possible ways this could have come about in an ancestor-manuscript, and these are just a few physical formats that would result in the exact same accidental omission:

Here's some more obvious examples from Matthew's gospel, in which the two 4th century uncials show the error came from a common ancestor, but remains an ancient blunder:

homoioteleuton (accidental line skips caused by similar endings of lines) is one of the most common scribal errors.

They get copied for multiple generations, because if the text still makes sense after the loss, they are very difficult to spot for correctors. By contrast, dittography (accidental repetition of a line) is easy to spot, and easy to repair (cross out the line).
