George O'Dowd went with the stage-name Boy (masculine) George (masculine) because he's not a man claiming to be a woman. The interesting thing with transgender men in Thailand is they don't call themselves women, they call themselves "lady-boys". Making it clear that while they like acting and looking female, they are indeed really male. They like Boy George are dealing with the reality that no matter how much they put into being feminine/female, they are in reality male.
