You're correct, he did. It caused controversy when he set up a go-fund me for heart surgery but refused to disclose exactly what he was using the funds for (there was 1.1 million unaccounted for). I don't know more than that as I personally didn't like him, but because of his struggle with heart issues, its probably more likely that heart disease just caught up to him.

I agree; a great loss. Was sad to learn of it.

I did some checking. So far, according to one article I just read, the cause of death and place of death haven't yet been disclosed to the public. All I've read so far is that it was an 'emergency medical incident'. I do think many are assuming it had to do with his heart.
