Any moral person who regards God and values His Word, knows that sex outside of marriage is wrong, and God forbids it. Adultery is when one adulterates the marriage. Yes, a Christian must not re-marry except in case of the death of their spouse, or when the spouse has re-married someone ---or otherwise is known to have been committing fornication.

Yes.

Infidelity: the word Jesus used doesn't mean "infidelity", it means impurity. In this context it specifically indicates marriage within certain degrees of kinship forbidden by Jewish law, such as the union between a man and his dead father's wife spoken of in 1 Cor. 5:1. Other examples would be unions between siblings or half-siblings, between parents and children of adoption, or between first cousins: see the meaning of the word consanguinity. This is also taken to include unions in which there was some other form of impediment at the time of contraction: marriage by force, for example.

Death? No, because marriage is dissolved on the death of one of the spouses. It's not possible to divorce someone who's dead, because there's no longer any marriage from which to be divorced.
