If you are Catholic, I would recommend frequent confession, daily examination of conscience, daily Mass attendance if possible.
Also, I would recommend fasting, you need to maintain control over your passions, and fasting is highly beneficial in this regard. Frequent prayer, always remind yourself of God's presence.
In addition, go to your priest, tell him of your struggles. Ask him if there are any support/accountability groups of which you could join, ask him for practical advice.
Consecrate yourself to Jesus through Mary, and daily pray the rosary.
Try to avoid idleness. If you are being tempted, do a selfless act, give of yourself, expend your energy in prayer and service.
Brother, I struggled with these same issues for about two years, and it was a battle. We must rely on God's grace and take the offensive. Sitting back, doing nothing, being idle, will do nothing to help you heal from this weakness, and will likely increase your dependency on this sin.
