Many of these things are part of "continuing revelation" (see: Sacred Tradition) and are not necessarily based on scripture.
The heirarchical structure is far more complex than the parameters you've allowed.

Another bonus to this system is you don't have to explain it, it just reads this way in the bible. One word you will not find in scripture is democratic or committee for that matter. This is a bone of contention I have with SB in general but all other matters outweigh this one, so I remain SB.
