Not necessarily. It could have been an attitude sensor ("gyroscope") failure, or failure in the circuitry from the sensor to the on-board computer. A software fault would be a design flaw. Worse yet, a lack of an adequate dynamics model of the rocket could have led to correct software programming of the wrong equations.
