Mostly personal self examining and prayer, but also the prayer in the liturgy for forgiveness and absolution play a large part. I take the words to heart and pray them with meaning, not just recite them with everyone.

Eastern Orthodox normally have regular confession, and might attend a Saturday evening service if offered in their area. The most regular part is to fast from the night before taking communion, thereby breaking the fast with the Eucharist. If a parishioner needs to eat with medication, that is normally permitted by speaking with the priest.

We're never worthy. One of the pre-communion prayers reads

And I, in my pitiableness, dare to receive Thy whole Body. Much of the liturgy leading up to the distribution of Communion is, by its nature, a preparation for communing (Creed, General Confession, Invocation, Prayer of Humble Access, etc)

So if you are participating in that (instead of sleeping in your pew, I guess )…

... it should constitute the major part of your preparation IMO.

You can read 1 Corinthians 11 about what was unworthy. Ones were eating and drinking, right next to ones who had "nothing". How we are relating with one another is very important to Jesus.

following a regular prayer rule, which includes the prayers of preparation for communion.

Exceptions and/or other specifics are based on individual need(s).
