There's a brief form tucked into the middle of most of the newer service books. I think it tends to be conducted as more of a private meeting in the pastor's office than the anonymity of the screened box.

I made my first confession last month shorty after this post was made. How often does one attend to this sacrament?

I always mention the availability whenever the liturgical color shifts to violet. If you desire to go more frequently, do so.
The Pastor mentions Holy Absolution often so I took him up on it and we are going to meet once a month for private confession.

In my Parish, you can just show up and request it, or call ahead. We do it kneeling at the rail in the chancel. Pastor standing on the other side of the rail. While it does not matter, where better place than kneeling before Christ's Altar.

We kneel at the rail, Pastor on the other side, and I use the words given in the LSB.
