Initial investigation blames Navy for USS Fitzgerald collision - CNNPolitics.com

Preliminary findings in the investigation into the collision between the USS Fitzgerald and a Philippine cargo ship off the coast of Japan in June suggest the accident was caused by multiple errors by the Fitzgerald's crew and a failure to take action in the minutes leading to the collision, according to two defense officials.

"They did nothing until the last second," one official said. "A slew of things went wrong." A second official said the crash "will wind up being our (the US Navy's) fault."

that captain's career is pretty much over.
i'm curious what these errors could be but suspecting boredom to have played a big roll in it. people make more mistakes when they are bored and not paying attention than when they are under some pressure.

there was another large vessel in the area, almost precisely paralleling the ACX Crystal, albeit displaced 2-3nm due north. The Fitzgerald may possibly have been focused on avoiding the larger, and nearer, "Wan Hai 266", and lost track of the Crystal beyond ?

Given the hour, and the cryptic crew comments to date, perhaps someone on the watch teams was literally asleep on duty ?

Maybe future vessels could be fitted with un-silenceable proximity alarms ? Perhaps bridge crews could manually insert muffles, if necessary, but the klaxons would be otherwise non-silenceable ??

Their report documents the routine, almost casual, violations of standing orders on a Fitz bridge that often lacked skippers and executive officers, even during potentially dangerous voyages at night through busy waterways.

When Fort walked into the trash-strewn CIC in the wake of the disaster, he was hit with the acrid smell of urine. He saw kettlebells on the floor and bottles filled with pee. Some radar controls didn’t work and he soon discovered crew members who didn’t know how to use them anyway.
