President Trump says: "I'm the only one that matters" in setting U.S. foreign policy, thus downplaying the importance of high-level jobs such as the assistant secretary of state, which is currently vacant.

Trump's massively inflated ego is larger than the size of Jupiter. He needs to take a breather from his obsessive self-aggrandizement and blow off some steam.

...I suspect he's so tone deaf to other people he doesn't so much "not care" what other people think he can't imagine other people have the "audacity" to hold opinions different from his own. And if they do? They're always going to be wrong.

He is right in that the final decision is his. But those officials are there to deal with the day to day to affairs of their respective bureau and to advise the Sec State and POTUS on issues that fall within their purview. Simply put, even though the ultimate responsibility is the President's, he just doesn't have the time or the attention to deal with those issues.
