If the chrism got going in the early days as a stand-in for the Apostles' laying on of hands...have bishops since the 1st century ever laid on hands rather than using the chrism?

Hmm... it is possible the Tikhonovskaya did so under the soviet yoke.

Any bishop can create the chrism, it's just normally done by patriarchs for the sake of affirming unity and order, but under circumstances like the gulag it was likely sometimes impossible to get your hands on any oil that could be used. So, I find it likely that laying of hands was done in such extreme circumstances. I feel like I've read about it being done that way under the Soviets, but I can't quite remember.
