I think it was a gradual evolution as the various colonies etc. became more autonomous in a variety of ways. In Australia, for example, the church was originally (as for all British subjects outside Britain) under the oversight of the Bishop of London; but in 1814 we came under the Bishop of Calcutta. In 1836 we became the diocese of Australia, but we weren't completely autonomous from England until 1962.
