This isn't about blaming anyone.
What hinders healing?
Unforgiveness "And whenever you stand praying, if you find that you carry something in your heart against another person, release him and forgive him so that your Father in heaven will also release you and forgive you of your faults." Mark 11:25. "And when you pray, make sure you forgive the faults of others so that your Father in heaven will also forgive you." MATT 6:14
Do you encourage them to forgive before you pray for them to be healed?
What are other things that hinders healing?
The sovereignty of God. Requesting healing from God and actually receiving it are two different things. We have control over what we can request of God, but we have no control over how and when the answer comes.
Where there is an absolute promise that applies to those who fulfill the conditions attached to it, such as exercising faith alone in Christ brings salvation, then as soon as the request is made, the promise is fulfilled and the sinner is saved. The promise, "I will be with you always, even unto the end of the world" is another absolute promise that applies without question to every true believer.
But there are promises that are not absolute and depend on the sovereignty of God and His providence. Healing and health is one of them. Healing is not guaranteed upon request. God has total control and the final decision whether a person is healed or not. He is not a vending machine in this regard, that someone can put a prayer in the slot and expect the answer to fall out the bottom like a can of Coke.
There are promises that are received through faith and patience. Many make a request for healing in faith upon the promise: "I am the Lord who heals you", but they forget about the patience part of receiving the outcome of the promise, and so when it doesn't happen within their time-frame, they lose faith in the promise. The promise does not say, "I am the Lord who heals you immediately you ask for it". For the true believer, if he or she dies before the healing comes, they are in Paradise with the Lord anyway, so it makes no difference.
For the unconverted sinner, if he requests healing, the Lord would answer, "Believe the gospel and receive Christ and be healed in your soul, heart and spirit", and once that happens, he is in the same position as the converted person, having to exercise faith and patience while awaiting the outcome which may or may not come.
So, we can blame the church, lack of holiness, lack of faith, etc. and so on, but really, it is all up to the sovereignty of God who decides, and there is nothing we can do about it except make our requests known to Him and to receive His peace while awaiting the outcome, whatever that may be.
Those who require God to heal immediately they request it are actually making God comply with their will, rather than resting in His will for them. The Scripture says, "Who do you think you are, trying to command Me?" said the Lord.
