I've also heard before that we will be able to ascent and descend to and from heaven, as heaven and earth will be sort of 'connected in a way.

Yes and no. We will...but they will be glorified bodies, as Christs was upon his Resurrection.
I believe so. New Jerusalem descends out of Heaven, to Earth. It seems to be a portal between the two realms, and while Earth will be our home, we will have access to Heaven.
