They would know which ones they are because their parents told them so based on their... oh wait, you said it wasn't about genitalia. That's my point. Kids go deep into their lives with the only way to even start to put them in a category is their genitalia. (Puberty will give other signals.) If kids didn't have genitalia that could be used for such assignment they could observe adults and might find themselves identifying with one or the other before their sex becomes apparent at puberty. Not true on both accounts.The only therapy and medication that have been shown to aleviate the suffering of those who have gender disphoria are gender-affirming treatments.
Any treatment and medication that focuses on trying to make the people suffering from gender disphoria adhere to their birth gender is extremely harmful and ineffective.
