Sunday in Lent song

At the parish I grew up in, every Sunday in Lent the priest would prostrate himself in front of the alter and we would sing this song during the penitential rite, this is as much as I remember from it:

Chorus:

I was kinda surprised that the parish at my school doesn't do it, cause I had always grown up with it. I'm not sure what it is, anybody know about this?
