Authorities are looking into how it happened. Officers found a 3-foot-by-4-foot cone-shaped piece of metal. Investigators determined it had fallen from a commercial airplane.
I hope this is an automatic review of all recent inspections. Because someone should notice a piece ready to "fall" off.

LOL - I was poking fun at the particular phrase. My bad.
You're right of course, having parts falling off a commercial jetliner is no cause for levity and should motivate a serious investigation of maintenance practices by the airline or its maintenance union for sure.
